A. INCOMING FIRST YEAR STUDENTS

REQUIREMENTS. Incoming first year Undergraduate students are required to submit the original copy of the following documents to the Office University Registrar.

  1. FORM 138 (Report Card)
  2. Certificate of Good Moral Character
  3. Certificate of live birth from NSO/PSA
  4. Marriage Certificate from NSO/PSA (for Married Female Students)
  5. Brgy Health Clearance
  6. 2x2 size ID Picture (white background)
  7. Long Brown Envelop

PROCEDURE. Incoming first year undergraduate students shall follow this procedure for enrolment.

  1. All qualified applicants shall submit the complete requirements to the University Registrar for recording and safekeeping. NOTE: In case, the student has no quarantine pass or unable to come to the University due to quarantine restrictions, he/she may authorize anyone from his/her parents, relatives or friends for enrollment purposes;
    1. Requirement:
      1. Authorization letter stating the name, relationship of the authorized representative to transact in his/her behalf.
      2. Student Applicant’s valid ID.
      3. Valid ID of the authorized representative
  2. The Office of the University Registrar shall issue the Certificate of Registration (COR) to manifest official enrolment in the University.

B. TRANSFEREES

REQUIREMENTS. Incoming first year students (Graduate and Undergraduate programs) are required to submit the original copy of the following documents to the Office University Registrar.

  1. Honorable Dismissal
  2. Official Transcript of Records (for evaluation/transfer)
  3. Certificate of Good Moral Character
  4. Certificate of GWA
  5. Certificate of live birth from NSO/PSA
  6. Marriage Certificate from NSO/PSA (for Married Female Students)
  7. Brgy Health Clearance
  8. Duly filled out Application for Crediting of Course
  9. 2x2 size ID Picture (white background)
  10. Long Brown Envelop

 PROCEDURE. Incoming transferee students shall follow the same procedure for enrolment.


C. SHIFTEE/RETURNEE

REQUIREMENTS. All shiftee/returnee students are required to submit the original copy of the following documents to the Office University Registrar.

  1. Duly accomplished Application for Shifter’s/Returnees form
  2. Duly accomplished Application for Crediting of Course form

 PROCEDURE. All Shiftee/Returnee students shall follow the same procedure for enrolment.


D. CONTINUING OLD STUDENTS

PROCEDURE. All continuing students shall follow the procedure for enrolment.

  1. The students enrolled in SY 2019-2020 shall be evaluated and enrolled in SY 2020-2021;
  2. The deans/campus directors shall designate the program chairs or class advisers to trace and document students' enrollment for this new normal time;
  3. The students shall confirm their enrollment through the college dean/campus director via mobile phone call or any mode of offline/online communication to discuss their specific courses to be taken. NOTE: The college deans/campus directors, by all means, shall find ways to reach out to their students;
  4. The college dean/campus director through the respective Program Chairs or Advisers shall prepare the academic advising forms upon confirmation of the students via phone call;
  5. The Academic Advising forms shall be forwarded to the University Registrar by different colleges for encoding of the courses to be taken in the system to ensure that all courses required/completion for his/her degree program can be completed this school year;
  6. The University Registrar shall print and forward the Certificate of Registration (COR) to the College Deans.
  7. The student shall get the Certificate of Registration (COR) at the Office of the Dean/Campus Director by appointment thru a text message which will serve as an entry pass in NwSSU campuses.

NOTE. Since students are given one (1) year to comply, the University allowed the enrolment of subjects with pre-requisite even without the grades yet but it does not mean the pre-requisites are waived. Students who will be given a chance to comply within a year must do so otherwise they will be given a grade of 5.0 or Failure. In addition, the pre-requisite subjects taken shall be declared null and void.
